By signing below, I acknowledge that I am voluntarily choosing to participate in athletic and recreational activities organized by JD TEAM and Joscar Duque Coaching. These activities may include, but are not limited to, running sessions, cycling rides, swim workouts, track training, triathlon camps, and other team events held in Miami, Florida or in other locations.

I fully understand that participation in these activities involves inherent risks, including the possibility of physical injury, illness, property damage, or even death. I certify that I am in good health, physically fit to participate, and solely responsible for evaluating my own limitations.

In full recognition of these risks, I hereby release, waive, and discharge JD TEAM, Joscar Duque Coaching, Joscar Duque, Maria Fernanda Pernalete, all affiliated coaches, staff, sponsors, volunteers, and partners from any and all claims, liabilities, or demands resulting from any incident, accident, or injury that may occur during or as a result of my participation.

I take full responsibility for my participation and agree to notify the coaches of any pre-existing medical conditions or limitations that could affect my ability to train safely.

Additionally, I grant permission for JD TEAM and Joscar Duque Coaching to use any photos or video footage taken during the events for promotional and social media purposes.

By completing this form, I confirm that I have read, understood, and voluntarily accept the terms of this release.
